The DVT Risk Assessment Screening was designed to promote a nationwide focus on venous thromboembolism (VTE) and pulmonary embolism (PE).  

The tool kit contains:

• VTE risk assessment
• DVT risk report card
• Patient education materials
• PR and marketing suggestions
• A step-by-step guide on holding an event

The VTE risk assessment screening program is a simple and easy questionnaire that allows health care providers to interpret risk for DVT or PE.  All participants receive educational materials about VTE and a report card to bring to their primary care provider.
